Payments:
All co-pays are due at the time of your appointment. We will send a statement with the remaining balance that is not paid by your insurance. All out-of-pocket expenses (deductibles and co-insurance) are due 20 days after the date of your statement.
For cosmetic procedures payment is due at the time of service.
