Schedule an Appointment
To make an appointment please call (805) 379-5970 or email us at info@prodermimage.com. We ask that you bring Photo I.D and insurance card with you to your first visit and arrive 5-10 minutes early to complete any forms/consents necessary. We ask that patients kindly give us a 24 hour notice if they cannot make their appointment. This courtesy enables us to better utilize appointments for our patients in need.
